How to use the genetic test sampling kit:

Stick the barcode on the genetic test retention card, the information sheet, the two specimen tubes and the return box, and give the retention card with the barcode to the examinee (for minors, please leave it to their parents) to facilitate the enquiry of the report in the future.
After the attention is completed, please ask the examinee (parent) to fill in the health information sampling form, use block letters to fill in truthfully and completely, and sign the form.
